Due to COVID–19 there are strict guidelines surrounding in-person patient visits at this time. Although these measures may seem extreme, they are designed to keep our patients and clinic staff safe and to prevent any further community spread.
We appreciate your understanding and cooperation. For current patients of STRIVE, or those considering us for their care, listed below is what you can expect with every visit.
All STRIVE team members utilize appropriate PPE at all times while in the clinic in addition to practicing frequent hand washing.
All patients will be required to complete a COVID 19 Screening Questionnaire. This will have been done prior to booking your appointment and again on the day of your visit.
At this time, we can only allow patients into the clinic. Family members or friends will not be permitted. Accommodations will be made for children or patients who require further assistance.
Patients will be asked to complete any necessary paperwork prior to their first appointment.
Please arrive 10 minutes before your scheduled appointment and be dressed in appropriate clothing for examination/treatment purposes.
Every treatment room is thoroughly cleaned and sanitized prior to every patient entry.
Our office door will remain locked until your scheduled appointment at which time our staff will call your phone. Please remain in/near your car until you are called. This is designed to keep the volume of patient traffic within the clinic as low as possible. Our staff will open the door for you and direct you inside. Please refrain from touching the door handle.
Masks are 100% mandatory to enter and remain in the clinic. Please wear your mask over your mouth AND nose.
Hand sanitizer is located immediately to the right of the door upon entering and in all exam rooms. We kindly ask that you sanitize your hands upon entering the clinic.
Please do not bring any unnecessary items or personal belongings into the clinic, including cell phones, purses, back packs. Please place your keys in your pocket.
Please use the washroom before coming into the clinic. The washroom will remain closed in order to minimize shared spaces and surfaces between patients.
Physical distancing will be respected at all times. Once your appointment has ended your physician or therapist will escort you back to the main door to avoid contact with other patients.
